Have your regular breast self-exams and other breast care must-dos


Early detection is one of the keys to survive breast cancer. Yet, many women need help remembering important things, such as doing a regular self-breast exam.

Yet, despite campaigns set by the health department and other NGOs related to women’s health, lack of awareness still remains the reason for poor compliance. Here are some frequently asked questions on breast care with answers courtesy of experts from the Manila Doctors Hospital.

When is the best time to perform a breast self-exam?
According to the American Cancer Society 2010, it is recommended once each month beginning at the age of twenty.

Breast Self-Exam (BSE) is done once a month so women may be more familiar with the usual appearance and feel of their breasts. Familiarity makes it easier to notice any changes in the breasts from one month to another. Some physicians suggest that women do their BSE every day for a month to make them more aware of how their breasts look and feel. Early discovery of a change from what is normal is the whole idea behind BSE.

The best time to do BSE is two to three days after the end of your period, when the breasts are least likely to be tender or swollen. A woman who no longer has periods may find it helpful to pick a particular day, such as the first day of the month, to remind her that it is time to do BSE.

How does one go about a BSE?
There is no rule of thumb when doing a breast self-exam. There are different techniques depending on what suits you best. Some opt to do it while facing a mirror, others while lying down, or while in the shower.

What’s the next step if you feel a lump in your breast?
If you discover anything unusual, such as a lump, a discharge from the nipple, or dimpling or puckering of the skin, you should see your doctor at once.

Many women have irregular or “lumpy” breast. The term “benign breast condition” refers to those changes in a woman’s breasts that are not cancerous. Many doctors believe that nearly all women have some benign breast changes after the age of thirty. But any of these changes is best consulted with your physician.

How does BSE affect us?

Breasts come in all shapes and sizes, just as women are. Our breast will change through time; hence, it is important for us to learn what is normal. For women, BSE is easy to do as the name implies.

Are there any other modalities?
Adult women should receive regular physician-performed clinical breast exams. It is recommended that women aged 40 years and above should undergo mammogram screening every one to two years. As soon as women reach the age of fifty, mammography should be performed every year.

There is no harm in being aware and getting involved in practicing and maintaining a healthy lifestyle. Develop a habit and engage your family, friends, and colleagues to do the same.

How singer Abby Asistio fought against alopecia and got her hair and groove back


Since she was four years old, singer and songwriter Abby Asistio has been suffering from alopecia areata, a type of hair loss condition that occurs when the body’s immune system mistakenly attacks hair follicles resulting in partial or complete loss of hair on the scalp and the rest of the body. Alopecia areata can be hereditary; it affects both men and women at any age.

Baldness, which is a result of the condition, can have its emotional toll on the patient. In a society where hair is considered an essential part of self-imaging, having thick hair in men and women is considered as added plus points in one’s level of attractiveness. Shampoo commercials, for instance, always play up having long and lustrous hair as vital in defining how a beautiful woman should look like.

Abby shared her own anxieties as a woman who was going bald due to alopecia, “I learned how to cope with having alopecia though I’ve been wearing wigs and hats. Iba yung feeling na maging malaya ka lang, be who you are, and not be judged for the way you look. Well, it came to a point that even if people were judging my appearance because of my condition, I was actually fine with it. I got used to having alopecia, na nakakalbo ako.”

About alopecia
At the Novuhair forum, guest speaker Dr. Ma. Rica S. Mallari of the Philippine Dermatological Society (FPDS) explained the hair loss condition.

Dr. Mallari said, “Alopecia areata is usually asymptomatic but sometimes there’s itching and burning sensation. It is common and is not difficult to diagnose, but sometimes it can be difficult to manage. Its causes are family history, stress (which is a major factor), and other medical illnesses like autoimmune disorders. What happens is the immune system of the person sees the hair follicle as a foreign object. So instead of attacking the virus and bacteria it attacks the hair follicle so the hair couldn’t grow properly, it falls off, and gets pulled off immediately. The aim of our treatment is to stimulate hair growth but we cannot alter the genetic susceptibility.”

Dr. Mallari added, “As we all know, hair is very important to all of us. So when you start losing hair, you become depressed. It covers a lot of cosmetic, psychological, and medical issues because sometimes when you have hair loss, there might be an underlying medical problem going on. You have to have it checked also. There are lots of causes of hair loss. It can be congenital (present from birth), acquired (obtained), focal (one bald-patch), diffused (all-over), non-scarring (you can grow your hair back), or scarring (you cannot grow back the hair because the follicle is already destroyed).”

During her talk, Dr. Mallari explained that hair loss has different causes. It can also be associated with the abnormalities of skin, nails, and teeth. Careful history and examination of the patient are needed. “We can do blood tests if there are some endocrine problems, diabetes, or thyroid problems.”

“Another is the hair pull test,” noted Dr. Mallari on another way to detect the reason for the hair loss. “We can also look at the hair under the microscopic equipment if there’s an abnormality in the hair shaft. There are times that we need to do a scalp biopsy to be able to make a definite diagnosis.”

Filipinos should closely watch their heart health, urges cardiologists


While February is Valentine month, romance is certainly in the air these days. But at Cardinal Santos Medical Center (CSMC), its medical experts are calling on Filipinos to take a closer look at their hearts—literally.

“There are four major risk factors for heart disease,” says Dr. Zenaida Uy, Chief Medical Officer for Cardinal Santos Medical Center (CSMC). “These are smoking, diabetes, hypertension, and high cholesterol. Outside of those, obesity as well as family history can also lead to cardiovascular complications.”

Identifying risks, however, is not the problem, explains Dr. Uy. She adds, “It is persuading those at risk to visit the doctor regularly. And in the event that they do visit, the bigger challenge is getting them to commit to a healthier lifestyle.”

Heart disease is deeply rooted in basic lifestyle choices—particularly in diet and exercise. Eating excessive amounts of red meat or smoking cigarettes ultimately take their toll in the long run, advises Dr. Uy.

“It is all about healthier habit formation,” she asserts. “Nonetheless, most Filipinos forego preemptive behaviors in favor of instant gratification. But once this horrid disease has taken hold of your body, everything changes; at this point, your past choices can no longer be undone.”

Heart disease costs over P500K
The World Health Organization (WHO) ranks coronary artery disease (CAD) as the leading cause of death in the Philippines. According to the National Statistics Office (NSO), cardiovascular diseases are responsible for at least five of 10 deaths in the country. In 2011 alone, this prevalent type of heart disease was responsible for 57,864 Filipino fatalities—that is over five times more than the death toll of Super Typhoon Yolanda.

“When a doctor suspects a patient has CAD, an angiogram is immediately recommended,” narrates Dr. Uy. “If critical artery blockages are found, that patient will need to undergo angioplasty or bypass surgery as well.”

According to Dr. Uy, both operations would cost around Php500,000 (easily more in severe cases). And if you consider medicines, she adds, a patient needs to spend Php5,000 to Php8,000 in order to manage the condition.

“And that is just financially,” warns the cardiologist. “The severe chest pains you have to endure and the emotional toll that burdens your friends and family cannot be expressed in Pesos. Worst of all, CAD—and heart disease in general—could be avoided.”

Commit to a healthy lifestyle

Forming habits centered on healthier choices go a long way in securing a person’s health and wellness. And Dr. Uy says this begins with “a well-balanced diet and an effective exercise regimen.”

She elaborates, “Low-fat and low-salt diets should be complemented by a moderate exercise routine at least 45 minutes a day, five times a week. You may also opt for more strenuous exercises at a lesser frequency.”

“Most Filipinos dread exercise, but it really depends on how you frame it,” notes the doctor. “I always advise couples to look at exercise as a bonding experience—an activity that not only maintains their health but also strengthens their relationship.”

She concludes, “Your body has a remarkable capacity to maintain your health. All you have to do is commit to healthier choices.”

Oral care: Window to overall wellness


MANILA, Philippines - Dr. Steve Mark Gan, one of the Philippines’ most respected dentists, opted to establish a dental practice here instead of in the USA where he had pursued advanced studies in Prosthodontic and Implant Education. Statistics show that 98% of the nation’s youth has dental caries, dental decay, or missing teeth. What people might not be aware of is that oral health can serve as a window to one’s overall wellness.”

“People generally think that the harder you brush, the better — when in fact, harder tooth brushing only leads to abrasions and gum recession. It is also not true that going to the dentist means setting yourself up for pain.”

The unflattering side effect of a low-carb diet


You’ve been wanting to lose weight so you can fit into that lovely red dress you’re eyeing to wear on your special date. So, you’ve decided to eliminate carbohydrates in your meal believing you will not gain extra pounds before the date. Yes, low-carb diets may be good for your waistline, but it may not be the same for your breath.

Bad breath from low or no-carb diets is often caused by chemicals called ketones that are released in the breath as the body burns fat. An example of this effect occurs especially in the popular Atkins diet. Carbohydrates aren’t readily available, so the body starts consuming other fats and proteins as its source of energy. As a result, one gets a breath problem.

It’s not an oral hygiene problem. Brushing, flossing, and scraping the tongue may not be able to hide the bad breath. Instead, reconsider your diet and modify it to avoid this embarrassing condition. There may be some ways to mask bad breath by (using mouthwash, for instance), but the basic problem still exists—so, better introduce complex carbohydrates (found in whole grains and vegetables like broccoli and spinach) in your diet instead.

What to do

Though this is not an oral hygiene problem, certain remedies can help banish the bad breath or at least mask the odor.

• Drink more water. Drinking water and swishing it around your mouth after your mouth after you eat helps moisten the mouth and gets the food particles that may contribute to odor, out. Chewing fresh parsley can also help.

• Chew sugarless gum or suck on sugarless mints. In particular, those that contain Xylitol also kill bacteria and can prevent cavities.

Optimum nutrition means having a well balanced meal—the greatest percentage for energy source should come from carbohydrates. The key is on the amount and kinds of foods that we eat. Complex carbohydrates may be good for dieters and with people who have diabetes because they are rich in fibers and makes one feel full at once. So, before you blame carbohydrates for any added inches in your waistline and decide to eliminate them altogether, think again. The downside could be unattractive, too.

Post-breast cancer surgery bras: Regaining confidence after mastectomy


A great number of Filipino women battle against breast cancer and this disease, is not only debilitating to the health but can also result to a social and psychological burden to some who have undergone full mastectomy or the complete removal of their breasts.

6 exercises to help ease your lower back pain


Everyone experiences lower back pains because of staying in one position for a long time, including sitting on one’s desk during office hours or staying on your feet for the whole day.

Physical therapists from Manila Doctors Hospital’s Department of Rehabilitation Medicine recommend these simple exercises for your lower back to soothe your tensed muscles.




1. Anterior-Posterior Pelvic Tilting
This exercise is meant to re-educate on pelvic awareness and strengthen the gluteus maximus.


Position
Supine lying with both knees bent and one hand placed beneath the back.

Technique
• Flatten the back against the bed by pressing on the hand.
• Hold the contraction for 6 seconds.

2. Knee hug/Knee to Chest
This is a flexibility exercise to stretch the tightened  muscles behind the leg and the back.


Position
Supine lying with arms at the side.

Technique
• Bring one knee to chest with hands clasped behind the  thighs; hold the position for 15 seconds.
• Progress the exercise bring both knees to chest with hands clasped behind both thighs.
Hold position for 15 seconds.

3. Sit-ups
This exercise strengthens the abdominals.


Position
Supine lying with both knees bent, arms at sides.

Technique
• Attempt to reach for both knees by lifting both shoulders partially from bed.
• Hold the position for 6 seconds if tolerated.

4. Toe-touch/Toe reach
This stretches the muscles behind the leg and lower back.


Position
Sit at the edge of a bed, sofa, or bench with one leg dangling.

Technique
• Slide both arms on legs in an attempt to reach for toes.
• Hold position for 15 seconds.

5. Forward Lunge Position
This is a flexibility exercise to stretch the hamstrings of the straight leg and to strengthen the thigh muscles of the bent leg.


Position
Place one leg forward, one leg behind with feet directed forward.

Technique
• Bend the forward leg without letting the knee extend beyond the tooes. The back leg should be kept straight.
• You are doing it right if you feel a stretch at the hamstrings muscles of the back leg.
Hold the position for 15 seconds.<

6. Wall sides
This flexibility exercise re-educates on pelvic awareness in standing and prevent tightness of back structures.

Position
Stand with the back against the wall with feet about 12 inches apart.

Technique
Flatten back against the wall. Hold this position and slide back against the wall while slowly bending both knees should not extend beyond the toes.
Hold the position for 6 seconds.
Perform each exercise twice a day with 10 repetitions per exercise. Count aloud when doing the exercise to prevent breath holding, and stop the exercise the moment pain increases in intensity.

'The Biggest Loser' coach weighs in with a weight-loss book


NEW YORK (AP) — Jillian Michaels orders two eggs over easy with a smidgeon of oil and two slices of dry toast.

And coffee.

Coffee?! "Two strong cups, 400 milligrams, fights pancreatic cancer," she says, "plus Alzheimer's, Type 2 diabetes and improves cognitive functions."

Not that Michaels is a health-nut goodie-goodie.

"I still drink a little bit of alcohol," she confides. "And I haven't been to the gym in five days!"

No wonder. There's this grueling book tour on top of an always-heavy workload, plus the routine demands of parenting a 3-year-old daughter and an 11-month-old son who, along with her partner, Heidi Rhoades, have come with her on this recent New York visit.




But all is never lost, says Michaels, in the battle to lose weight and be healthy. "Even if you're just standing while you're talking on the phone," she offers, "you can burn up to 300 calories in a day."




That's the sort of forgiving advice found in her latest book, "Slim for Life: My Insider Secrets to Simple, Fast and Lasting Weight Loss" (Harmony Books).

"It's my softest approach to weight loss," says Michaels, a wellness coach to whom the word "soft" is seldom applied.

After all, she is famous as the drill-sergeant trainer on NBC's "The Biggest Loser," a 5-foot-2-inch force of nature who doesn't hesitate to throw her tautly muscled weight around.

But during this recent breakfast she seems different from her "Loser" persona. Clad in jeans, sweat shirt and Ugg boots, her hair pulled under a newsboy's cap, she could pass for half her 39 years. She is animated, high-rev. But no way overbearing.

"I wanted to write a book where you felt like I was sitting right there with you," she says, a vision of reassurance seated across the table, "providing a simple solution for every problem or complaint I've ever heard."

Fitness is too time-consuming, complicated, costly, inconvenient, plus I'm hungry all the time — Michaels has heard every excuse from the audience for her website, weekly podcast and speaking engagements.

"I wanted to integrate the answers and knock down the myths and the fad diets," she says. "For every possible dieting dilemma that you could ever have, I provide umpteen amount of solutions. Pick one!"

In her book, every strategy comes with a point system scored from 1 (a "bonus" tip) to 3 (most effective and important). Totaling the strategies you're able to adopt can help predict your rate of weight loss, she says.

If some of this stuff gets a little technical (she prescribes workouts complete with calories-per-minute burned for each exercise), Michaels also packs the book with simple no-brainers: Eat before you head to the party so you're less tempted by those fatty hors d'oeuvres. Nix foods tagged with "danger words" like smothered, loaded, tender, deep-fried and creamy. At the supermarket, avoid the center aisles (high-trafficked destinations for junk food, she warns) in favor of the store perimeter, where fresh foods are likely to be stocked.

For imbibers who aren't satisfied with the occasional red wine (pretty healthy in moderation), she even offers recipes for low-cal cocktails.

"I'm going to show you exactly what you need to understand, exactly what never to do, and what it looks like in your life," she says. "This is never going to be easy. But it's never gonna be easier than this."

Growing up, physical health wasn't something that came easily to Michaels.

Her dad was overweight, she says, "and one of the ways that we spent time together was through food: 'Let's go get a pizza.'"

Her parents went through what she calls an ugly divorce when she was 12, which only hardened her image of herself as "a fat kid, a loser, someone who deserved to get picked on."

But a few years later she got hooked on martial arts. She had long felt like an outsider in school and most everywhere else, a feeling heightened by the fact that she was gay and hadn't yet accepted it. But here in the dojo she was part of a community. She felt supported. She blossomed.

Then came a real turning point: She broke two boards with a sidekick.

"The next day when I walked into the school, no one ever (messed) with me again," she says, her eyes blazing at the memory.

From there a career unfolded for Michaels as a trainer, physical therapy aide, then sports-medicine professional.

A decade ago, she signed on to "The Biggest Loser." There, instantly, she stood out as a taskmaster, even a bully.

"I always identify with the underdog, and I think that's one reason I feel fine yelling at them," she explains. "I feel like I'm yelling at a peer: Take responsibility, own this situation and bring your best. Let's start exploring your potential."

As "The Biggest Loser" heads toward its season conclusion (Monday at 8 p.m. EDT), Michaels has seen full potential reached by her current charge, Danni. A 26-year-old advertising account coordinator from Wheeling, Ill., Danni has lost 95 pounds under Michaels' dogged coaching and has guaranteed herself a slot as a finalist.

"You found yourself and you just soared," Michaels told her last week in a voice choked with emotion, "and you became everything that I had hoped you would be."

Michaels returned to "The Biggest Loser" this season after a two-year absence. Her reasons for coming back included "a whole new group of producers I really trust and like," she says. "Besides, it's a heckuva platform."

But it's only one of many platforms from which this go-go fitness guru spreads her gospel — a gospel she says isn't really about fitness.

"It's never been about fitness for me," Michaels says. "I don't even really like to work out. But when you're strong physically and you feel confident about your body and your health, you're strong in every other facet of your life. It's transcendent."

When you’re up in arms against sweat

I’m an avid reader of your column and decided to consult you about my problem. I’m an 18-year-old student and I have been experiencing sweating in my underarms. I’ve tried using different brands but to no avail. My underarm sweats all the time even if I am not doing anything and worse, the sweat leaves stains on my white uniform. It’s very uncomfortable and I’m starting to lose my confidence. Any effective anti-perspirant you can recommend? It would be great if it is affordable, too, since I’m on a tight budget.

Please try Dove Minimising Anti-Perspirant deodorant spray. This affordable drugstore brand may be known to be gentle but this softie is a toughie on sweat! True story: One time, my husband ran out of his favorite men’s deodorant so I offered the Dove deo spray sitting on my bathroom shelf. He was so surprised it kept him dry and sweat-free throughout an entire day, even after hours of being in a hot and humid basketball gym! Men generally sweat more than women, but this Dove deo spray was able to handle it!

As for the sweat stains on your uniform, they say you should never use bleach on a perspiration stain, especially on white cotton fabrics. The chlorine in the bleach will react chemically with the proteins in sweat and cause the stain to set even darker! One friend of mine tried using bleach, and the pale yellow stains turned almost mustard in color! Also never iron or put a stained garment in the dryer — any heat will set the stain permanently.  Do not dry or heat the garment until the stain is gone. Hope these tips will be of help. Good luck!

Five tips to silkier skin


MANILA, Philippines - Slithery, mysterious and sleek, the snake is known for many distinctive traits. But in this Year of the Snake, there’s something we don’t look forward to: scaly skin.

So to start the new year right, exfoliate all that negative energy from the previous year to start fresh. Here are some tips on how to achieve smoother, silkier skin:










1. Since our skin regenerates every second of the day, dead skin accumulates. When showering, use a cleansing soap before a moisturizing soap. Cleansing soaps “strip” the body of perspiration, excess oil, dirt and, to some extent, dead skin.

2. Use a moisturizing soap after to help bring back and retain moisture to the skin that may have been rubbed during the cleansing process.

3. Depending on how dry your skin is, exfoliate using scrubs, wet hand towels, pumice stone or exfoliating gloves (whichever is your preference) once or twice weekly. This helps remove a good amount of dead skin that causes roughness, dullness and even ingrown hair on arms and legs.

4. Try using anti-dandruff shampoo as a body wash to help lift and remove dead skin. Leave for a minute or two before washing off.


5. Do NOT completely towel dry yourself. Instead, using any type of body oil or baby oil, apply on damp skin to lock in or seal optimum moisture. Rub in completely until oil is fully absorbed.

So whatever sign in the Chinese zodiac you may be, rest assured it’ll be smooth-skin sailing for 2013!

We all go through blah days. Our skin has a cycle of growing new cells and casting off the old. When dead cells accumulate, they make the skin’s surface look dull and gray or even rough. The Cleansing Powder comes in a black handy sachet — perfect to slip into your kikay kit and for traveling. It claims to clean and exfoliate simultaneously with the use of fine grains that work on the surface and jojoba to deeply clean. All you have to do is pour the powder into the palm of your hand and add drops of water, then massage onto damp skin. I liked that it didn’t contain any harsh fragrance. The powder dissolved into almost nothing as I massaged it in circular motions, so I frankly didn’t really expect much exfoliation. But the results are jaw dropping: I saw a visibly brighter complexion and skin as smooth as a newborn’s! It felt so smooth that I kept touching my face over and over again! Would I buy and use it again? Definitely. Apart from providing a deep clean, it revives dull and tired skin in an instant! It’s perfect if you have a big date, a wedding or an important event to attend. I’m all praise for this wonder cleanser. Now I can’t wait to check out their other products, too!

Taking the heat off yoga
